    LiveWell Chronic Disease Management (CDM)

    LiveWell with Chronic Conditions


    Who is it for

    Caregivers and/or individuals who have chronic health condition(s) such as: chronic pain arthritis, diabetes, heart disease, chronic lung disease, Parkinson's disease, stroke/spinal cord injury, osteoporosis, multiple sclerosis, and kidney disease.

    Goals

    To help participants obtain information, learn new skills and abilities, and develop higher levels of confidence to manage and cope with chronic health conditions.

    To give people more confidence in handling their own health problems.

    To give and receive support from others who are experiencing similar health problems.

    Service

    Included in this six week program are:

    • Problem solving and action plans
    • Exercise
    • Cognitive symptom management
    • Nutrition
    • Fatigue and symptom management
    • Advanced care directives
    • Use of medications
    • Dealing with fear, anger and depression
    • Communication with others

    Additional

    The program began in 2002 ,and it is delivered under license from Stanford University by volunteer lay leaders. Pairs of trained volunteers work with groups of up to 12 people, once a week for 2.5 hours for six consecutive weeks. Results suggest that the program impacts people's sense of well being and reduces healthcare utilization.

    Leader Training Opportunities

    We are always looking for volunteer peer leaders to co-lead this six week program.  Leaders receive four full days of training as well as a leader’s manual and would ideally commit to leading a minimum of 2 six week sessions.

    Living Well with Chronic Disease

    Exercise Program

     

    Who is it for?

    The program is intended for individuals with

    • any type of chronic disease (eg. arthritis, COPD, diabetes, high blood pressure, heart disease, stroke, etc.) or
    • any risk factor for coronary artery disease (eg. high cholesterol, overweight, etc.).

     We also encourage participation of a support person (spouse, family member or close friend).

     

    What happens at the program?

    Activity options include walking, strength training (using both hand weights and Therabands) and stretching in a safe and social environment. The program offers both individual and group exercise and education. Health professionals will be present on occasion to answer questions and help you develop healthy lifestyle habits as you participate in the program.

    Program Goals

    To provide a safe, supportive exercise environment and a high quality prevention/rehabilitation program for participants and their families who want to take control of their health and future well-being, return to optimal levels of physical, psychological, vocational, and social abilities, and adopt healthy lifestyle habits.

    What to bring?

    • Comfortable clothes to exercise in.
    • Clean and comfortable shoes
    • Water bottle.

    Program Times

    Outlook – Walking every Monday, Wednesday and Friday from 8:30 -10:00am; group stretch/strength class 9:15am

    Beechy – Walking every Tuesday and Thursday from 1:00-2:00pm; group stretch/strength class 1:40pm

    Lucky Lake – Walking every Tuesday and Thursday from 9:15-10:45am; group stretch/strength class 10:00 am

     

    Heart to Heart Program

    The Heart to Heart program is a 6 week education and support program of the Heart and Stroke Foundation of Canada that assists cardiac patients and their families who have, or are at high risk for cardiovascular disease.

    Education takes part in a small group setting.  The group meets once a week for 6 weeks for 1.5 - 2 hours at at time. 

    Each week features a different guest speaker.  Information is presented by Nurses specializing in cardiac care, Pharmacists, Dieticians, and Physical Therapists.

    Session topics include:

    Meet Your Heart
    Cardiac Procedures, Treatments, and Medications
    Eat Heart Smart
    Physical Activity
    Stress and Stress Management
    Community Resources

    Due to the small nature of the groups, there will be plenty of opportunity to ask questions during the sessions. 

    Spouses, family members, friends, or other support persons are welcome to attend as well.

    The program is free of charge.
