Teams

No different than a hockey or football team, a team of health care providers requires the efforts of all of its members to meet the health needs of patients and clients, and also communities as a whole, in order to be a success.

Local primary health care teams demonstrate a team approach on a daily basis involving communities and individuals in the decision-making process. Primary health care teams also see health care providers working together to:
  • improve the continuity of care
  • reduce duplication of service
  • ensure individuals have access to appropriate health professionals.
Whether as a patient or client, nurse practitioner or physician – we are all a part of our primary health care team.
