Information

Information and communication are a very important part of primary health care. This key piece of the primary health care puzzle involves not only increased communication between health care providers, but it also encourages patients to have a greater understanding of their health.

Learning more about the health care resources available to you in the Region and taking a proactive approach to your health will assist in taking better care of yourself and helping you make the right decisions about your health. It will also make communicating with your care provider easier and ensure you are getting the most from the health care system.


Through channels such as the HealthLine and the annual Diabetes in Focus Day the Heartland Health Region is encouraging you to take ownership of your health by providing you with the health care information you need to stay informed and in control of your health. Working together, it is easy to see that primary health care is Heartland’s “Winning Combination”.
